Insurance Requirements:

A Certificate of Insurance (COI) listing the relevant policies (GL, WC, Auto, etc.), including the following entities as certificate holders; TerraNova Enterprises, LLC and Nova 31625, LLC with the legal address 1301 6th Ave West; Bradenton, FL 34205 with the included verbiage: “Certificate Holder is included as Additional Insured, as required by contract, including its subsidiaries, affiliates, related entities, ATIMA.”

COI must be sent by your insurance agent to Rachel@TerranovaEquestrian.com once payment is accepted.

Julie Mullins will reach out to discuss boutique setup timing and requirements once your form has been submitted.

Vendor Agreement Form

TerraNova Equestrian Center's Vendor Form may be executed by electronic signature in accordance with Florida law, and electronic signatures appearing on this agreement are the same as handwritten signatures for the purposes of validity, enforceability, and admissibility.

Release and Waiver of Liability, Assumption of Risk and Indemnity.

TerraNova Equestrian Center's Release and Waiver of Liability, Assumption of Risk and Indemnity may be executed by electronic signature in accordance with Florida law, and electronic signatures appearing on this agreement are the same as handwritten signatures for the purposes of validity, enforceability, and admissibility.
